This study demonstrates for the first time that vasoconstrictive effects of physiological concentrations of insulin during inhibition of NOS activity are mediated by endothelin and that insulin induces endothelin-1-mediated vasoconstriction in isolated skeletal muscle arterioles during inhibition of PI3-kinase.

Endothelin has been shown to stimulate oxidative stress by causing up-regulation of the subunits of NAD(P)H oxidase and stimulating production of superoxide (Duerrschmidt et al., 2000). The effects of endothelin receptor blockade on the pulmonary circulation have been reported variably, possibly in relation to a more or less important associated release of endogenous nitric oxide (NO). The aim of this study was to test whether endothelin antagonism would inhibit hypoxic pulmonary vasoconstriction, and if it would not, then would it do so after NO synthase inhibition. In view of this profile of biologic action, it seemed plausible that endothelin could be involved in the development of the functional vascular disorder in scleroderma. Endothelin-1 (ET-1) is an endothelium-derived vasoconstrictor peptide that contributes to basal vascular tone. Endothelin-3 caused transient forearm vasodilatation at this dose, whereas endothelin-1 showed only a nonsignificant trend toward causing early vasodilatation.

Although some patients do well with calcium channel blockers, most ultimately need more advanced therapy, such as prostanoids. Recently, a new class of therapeutic agents has been developed to treat these patients: the endothelin receptor antagonists (ERAs). ET A is a subtype for vasoconstriction These receptors are found in the smooth muscle tissue of blood vessels, and binding of endothelin to ET A increases vasoconstriction (contraction of the blood vessel walls) and the retention of sodium, leading to increased blood pressure. Endothelin-1 (ET-1) is a 21 amino acid peptide that is produced by the vascular endothelium (click here for details). It is a very potent vasoconstrictor that binds to smooth muscle endothelin receptors, of which there are two subtypes: ET A and ET B receptors. Endothelin type A receptors potentiate the vasoconstriction accompanying angiotensin II infusion or blockade of NOS. Endothelin is released by hypoxia, specific agonists such as angiotensin II, salt loading, and cytokines. Nonspecific blockade of endothelin receptors lowers BP in models of volume-expanded hypertension.
